PARENT FORMS AND INFORMATION

CHANGE OF ADDRESS, PHONE, EMAIL ADDRESS, MEDICAL OR CUSTODY

To update your child's information, please click this link.  Please contact your building secretary if you have issues logging into FinalForms. You may enter in a change of address, phone number, email address, medical or custody information. 

Families who are doubling up with another Stow or Munroe Falls resident may use the Sworn Statement of Residency as a proof of residency coupled with the resident's utility bill, lease or home deed.

STUDENT WITHDRAWAL

Parents who wish to withdraw their child(ren) from the Stow-Munroe Falls City Schools may contact the school office of their attending building to withdraw their student(s). Parents may fill out the Student Withdrawal Form in advance and bring along to expedite the process.

STUDENT RECORDS RELEASE FORM

This form allows parents and guardians to authorize the sharing or transfer of student educational records. To ensure privacy and compliance, written consent is required before student information can be released.

STUDENT OPT-OUT REQUEST FORM

This form enables parents and guardians to exercise their rights under various state
and federal laws to opt out of their children’s participation in certain programs or activities.

BEFORE/AFTER SCHOOL CARE

The Riverfront Family YMCA provides before and after-school childcare at the following Stow-Munroe Falls schools: Echo Hills, Fischcreek, Highland, Riverview, Indian Trail, and Woodland. Before care begins at 7:00 a.m. and pick-up is by 6:00 p.m. The program is located in the school gymnasium.
 
Rates for the 2025-2026 school year:
 
Before Care
  • YMCA Member Rate: $65 per week
  • Non-Member Rate: $75 per week  
After Care
  • YMCA Member Rate: $75 per week
  • Non-Member Rate: $85 per week
Before and After Care
  • YMCA Member Rate: $100 per week
  • Non-Member Rate: $110 per week
Daily Rates
  • Before OR After Care: $30 per day
  • Before AND After Care Daily Rate: $40 per day
Required Registration Fee
  • A non-refundable/non-transferable registration fee of $40 is required.
