Required Forms

There are 5 forms that need to be completed and returned in order to participate in athletics at Kimpton Middle School.  Please click on the links below, print, complete and return to your son's/daughter's coach.

2.  Activity Fee Letter and Payment Form

3.  Athletic Participation Form

4.  Emergency Medical Card

5.  Physical Form ***A physical is required to participate.  A physical is valid for one year.  For example:  If the physical was completed on 6/1/12 - it will be good until 5/31/13.
